چکیده

In this paper is described a technique for performing tibiotalocalcaneal arthrodesis using a new type of intramedullary fixation, the ankle fusion nail. This paper also contains a retrospective review of 17 patients with a mean age of 46 years who underwent tibiotalocalcaneal arthrodesis with intramedullary fixation for diagnoses of osteoarthrosis (12 patients), rheumatoid arthritis (4 patients), and destructive distal tibial giant cell tumor (1 patient). With a mean follow-up interval of 22 months, an 82 percent union rate was achieved at a mean time to union of 14 weeks. The authors of this paper describe their technique for accomplishing this form of ankle and hindfoot arthrodesis with an intramedullary ankle fusion nail, and conclude that intramedullary nailing is a solid method of fixation for tibiotalocalcaneal arthrodesis in the appropriately selected patients. The best clinical results (100 percent union rate, 0 percent complication rate) after tibiotalocalcaneal arthrodesis with intramedullary fixation have been obtained in nonneuropathic patients undergoing their first fusion procedure for diagnoses of primary osteoarthrosis or rheumatoid arthritis.

Intramedullary rod fixation compared with blade-plate-and-screw fixation for tibiotalocalcaneal arthrodesis: a biomechanical investigation.

BACKGROUND Achieving stable fixation when performing tibiotalocalcaneal arthrodesis can be challenging, especially in osteopenic bone. The purpose of the current investigation was to compare the stiffness and fatigue endurance of blade-plate-and-screw fixation with intramedullary rod fixation in a cadaveric model. METHODS In ten matched pairs of fresh-frozen cadaveric legs, a tibiotalocalcane...

Ankle arthrodesis for failed total ankle replacement.

Between 1999 and 2005, 23 failed total ankle replacements were converted to arthrodeses. Three surgical techniques were used: tibiotalar arthrodesis with screw fixation, tibiotalocalcaneal arthrodesis with screw fixation, and tibiotalocalcaneal arthrodesis with an intramedullary nail. As experience was gained, the benefits and problems became apparent. Successful bony union was seen in 17 of th...

Tibiotalocalcaneal Arthrodesis Using a Femoral Locking Plate

The goal of a tibiotalocalcaneal arthrodesis is to create a pain-free, stable hindfoot and ankle. Although a reserved procedure, it is useful when simultaneous ankle and subtalar joint pathology exists. Numerous complications have been reported after tibiotalocalcaneal arthrodesis, most importantly nonunion. Locking plates have proved to be a more stable construct than alternative forms of arth...

The success of tibiotalocalcaneal arthrodesis with intramedullary nailing--a systematic review of the literature.

Tibiotalocalcaneal arthrodesis with an intramedullary nail may find indications for a variety of conditions of the hindfoot. We performed a systematic review of the literature primarily to find out the union rates and complications of tibiotalocalcaneal arthrodesis with an intramedullary nail. We included 33 studies for this review. These studies included 631 patients who had 659 procedures. Ei...

Arthroscopic tibiotalocalcaneal arthrodesis in neurological pathologies: outcomes after at least one year of follow up.

The main complications of open tibiotalocalcaneal arthrodesis are wound healing disorders and nonunion. Our hypothesis was that arthroscopy and interlocking intramedullary nailing decrease these complications. We retrospectively reviewed six patients (mean age: 58 years; mean preoperative Kitaoka score: 51/100) having undergone arthroscopic tibiotalocalcaneal arthrodesis with retrograde intrame...
